We are looking for positive outgoing person to provide direct care services at our day program Monday – Friday (no weekends, no holidays) hours 9:00am - 2:00pm.

Direct Care - PT (Lake County (Mentor))

Applicants must:

* Reliable and punctual with consistent attendance.

* Ability to read, write, and understand English

* Clean background check. BCII (Ohio) and FBI (if not a 5-year Ohio resident) checks are mandatory, checking for disqualifying offenses.

* High School Diploma or equivalent.

* Reliable transportation (MUST)

* Ability to work independently and as part of a team

Key Responsibilities:

* Support individuals with daily activities, life skills, and personal goals

* Facilitate group activities, recreational programs, and community outings

* Assist with personal care as needed, following individual service plans

* Ensure a safe, supportive, and respectful environment

* Accurately document daily services and activities per Ohio DODD requirements

* Communicate effectively with supervisors, coworkers, and families

* Follow all agency policies and state regulations

Here at AUS we are off every evening and on weekends. We offer paid federal holidays off and also shut down for (2) weeks each year (week of July 4th and Week of Christmas/New Years). These weeks are unpaid except for holidays.
